Bring a copy of your insurance card and a legal form identification to your appointment. Acceptable forms of ID include a driver’s license, social security card, or birth certificate.

You may also want to bring a list of any current or past prescription drugs or medical charts from your doctor to assist in the evaluation process. In the days leading up to your appointment, write down how you feel and any changes in your symptoms. You can expect your evaluation to last one hour. During this time, you will meet one-on-one with a qualified therapist to discuss your background, medical history, family history, and any abnormal changes in your behavior or thinking styles.
